1) SAWP E.A. Annex – to be filled out and signed by both the employer and worker prior to the worker’s first day of work.

Mexico LMIA’s – You will receive both an English and Spanish version of the above document.

CRITICAL INFORMATION – It is extremely important that the information you fill out on the E.A. Annex is EXACTLY what you stated on your approved LMIA.

Please note that the wage listed on the annex will state what the wage was at the time you placed your LMIA with F.A.R.M.S. You must edit the annex you receive to the current minimum or SAWP wage, if necessary.

2) F.A.R.M.S. Employer Vacancies Summary– Use this worker name list to validate the workers currently employed, or request your named workers for the current season. Write the requested arrival date beside each worker name and upload the list with your LMIA Online Application. Do not manually enter SAWP worker names in the online portal.

3) LMIA Requested Travel Dates – This report is a summary of the workers requested on the LMIA Online application  as well as the anticipated arrival and departure dates or transfer dates. This report is not a summary of arrived workers. Please choose the F.A.R.M.S. Employer Vacancies Summary to view arrived/transferred workers.
